Document & Visa
1. Paspor asli baru dan lama
- Masa berlaku 6 bulan keatas
- Paspor wajib ditandatangan di bagian kotak halaman paling belakang
2. Pas foto berwarna ukuran 3,5 x 4,5 cm dengan latar belakang putih (di zoom 70 %), sebanyak 2 lembar
- Foto wajib foto terbaru
3. Surat Sponsor (dalam bahasa Inggris)
- Di atas kop surat perusahaan
- Ditujukan untuk Kedutaan Selandia baru
- Dicap/stempel (apabila tidak ada, mohon tandatangan di atas materai 6000)
- Tidak boleh mensponsori diri sendiri
4. Fotokopi SIUP (jika Pemilik Perusahaan)
5. Fotokopi bukti keuangan 3 bulan terakhir
6. Fotokopi Kartu Keluarga
7. Fotokopi Akte Nikah
- Apabila sudah cerai lampirkan Akte perceraian
- Apabila pasangan sudah meninggal, lampirkan akte kematian
8. Fotokopi Akte Lahir Anak (jika ada anak yang ikut serta atau disponsori oleh anak)
9. Fotokopi KTP
10.Surat Izin Suami/Orang tua + fotokopi KTP (jika suami tidak ikut bepergian atau jika anak pergi dengan salah satu orang tua)
- Surat izin ditandatangani di atas materai 6000
11. Fotokopi Surat ganti nama (jika ada dan diterjemahkan ke bahasa Inggris)
12. Fotokopi Kartu Pelajar (jika ada anak yang ikut dan masih bersekolah)
13. Reservasi penerbangan internasional
14. Formulir harus ditandatangani di halaman 6 pada kolom “Signature of Principal Applicant“ (jika istri dan anak di bawah 20 tahun ikut bepergian, masing-masing tanda tangan pada formulir yang sama pada 3 kolom di bawahnya
15. Untuk anak di atas umur 20 tahun formulir sendiri ditandatangan terpisah dari keluarganya
Note:
- Jika Kartu Keluarga terpisah maka Akte Nikah, Akte Lahir anak dan Surat ganti nama harus diterjemahkan ke bahasa Inggris
- Jika nama berbeda di Kartu Keluarga, Paspor, Akte Nikah, Akte lahir atau rekening pribadi, harus melampirkan surat keterangan Lurah dalam bahasa Inggris
- Persyaratan di atas bisa saja bertambah atau berubah sewaktu-waktu karena disesuaikan dengan kondisi kelengkapan dokumen-dokumen yang diserahkan pemohon visa
1. New and old original passports
- The Passport validity min.6 months
- The Passport must be signed on the back of the box
2. Passport color photo size 3.5 x 4.5 cm with a white background, face zoomed in at 70% for 2 sheets
- Photos must be recent and have never been used on other visas
3. Sponsor Letter (in English)
- On company letterhead
- Addressed to the Embassy of New Zealand
- Stamped/stamp (if not available, please sign on 6000 stamp duty)
- Not allowed to be self-sponsored
4. Copy of SIUP / Business License (if business owner)
5. Photocopy of Financial Reports for the last 3 months
6. Photocopy of Family Card (translated into English)
7. Photocopy of Marriage Certificate (translated into English)
- If you are divorced, attach a Divorce Certificate (must be translated into English)
- If the spouse has passed away, attach a Death Certificate (must be translated into English)
8. Photocopy of Child’s Birth Certificate (if there is a child participating or sponsored by the child and must be translated into English)
9. Photocopy of ID Card (must be translated into English)
10. Husband/Parent Permission Letter + photocopy of ID Card (if the husband does not travel)
- The license is signed on stamp duty 6000
- If the child is traveling with one of the parents, the permission letter must be from a notary
11. Copy of Declaration of Name Change letter (if any)
12. School certificate (if there are children attending and still attending school)
13. International flight reservation
14. The form must be signed on page 6 in the “Signature of Principal Applicant” column (if the wife and child under 20 years are traveling, each signature on the same form in the 3 columns below
15. For children over the age of 20, fill out a separate form from their families
Note:
- If the family card is separated, the marriage certificate, child birth certificate, and name change certificate must be translated into English
- If the name is different on the Family Card, Passport, Marriage Certificate, Birth Certificate, or personal account, a letter from the Head of the Village must be attached in English
- The requirements above may be added or changed at any time because they are adjusted to the condition of the completeness of the documents submitted by the visa applicant
