1. New and old original passports

• The Passport validity min.6 months
• The Passport must be signed on the back of the box
• If you are still a minor and cannot sign, you must have an “Unable to Sign” stamp from immigration.

2. Passport color photo size 3.5 x 4.5 cm with a white background, face zoomed in at 70% for 2 sheets

• Photos must be recent and have never been used on other visas

3. Sponsor Letter (in English)

• On company letterhead
• Addressed to the Dutch Embassy
• Stamped/stamp (if not available, please sign on 6000 stamp duty)

4. Copy of SIUP / Business License (if business owner)

5. Photocopy of Financial Reports for the last 3 months

6. Photocopy of Family Card (translated into English)

7. Photocopy of Marriage Certificate (translated into English)

• If you are divorced, attach a Divorce Certificate (must be translated into English)
• If the spouse has passed away, attach a Death Certificate (must be translated into English)

8. Photocopy of Child’s Birth Certificate (if there is a child participating or sponsored by the child and must be translated into English)

9. Photocopy of ID Card (must be translated into English)

10. Husband/Parent Permission Letter + photocopy of ID Card (if the husband does not travel)

• The license is signed on stamp duty 6000
• If the child is traveling with one of the parents, the permission letter must be from a notary

11. Copy of Declaration of Name Change letter (if any)

12. School certificate (if there are children attending and still attending school)

13. International flight reservation

14. Hotel reservation in the country to be visited

15. Travel insurance

16. For children under 19 years of age, both parents must be present during the biometric process

17. All application participants, including children, must attend, even if they have had fingerprints/biometrics previously
